SUBMIT APPLICATION
Please submit your completed application along with the $50.00 application and testing fee (made payable to Wellness Society USA*) by either:
- Uploading the application and paying the $50.00 fee below, or
- Mailing the application and check to Dr. Sharon Fair, 200 Silver Lake Terrace, Palatka, FL 32177
* Wellness Society USA, Inc., a 501(c)(3) nonprofit, is doing business as Silver Lake Academy. All payments should be made payable to Wellness Society USA.
PAY APPLICATION & TESTING FEE
DIAGNOSTIC (PLACEMENT) TESTS
- Your child must complete Diagnostic (Placement) Testing in English and Math and an abbreviated IQ test so we can enroll him or her in the appropriate level coursework within the A.C.E. PACEs framework.
- The tests are administered over four or five school days so the student may complete them at a comfortable pace.
- All testing is conducted at the administrative office located at 200 Silver Lake Terrace, Palatka.

FLORIDA STATE DOCUMENTS
So we may legally enroll your child in Silver Lake Academy,
please submit copies of the following FL state documents for your child:
- Birth certificate. (And, if indicated, custody/guardianship paperwork.)
- State of Florida School Entry Health Exam (Form DH 3040) dated within the past year.
- The Florida Certification of Immunization, Form DH 680. This form is not available to the public and must be completed by a licensed Florida physician or nurse practitioner (NP), or a Florida county health department. Note: If your religious convictions conflict with this requirement then you may submit Form DH 681 (Religious Exemption).
